Record the scope and limitations
Identify the standard, organisation boundary, evidence reviewed and areas not covered. A limited sample should not be presented as a complete review of everything.
Separate the observation from the action
Explain the observed gap before deciding how to address it. The organisation may have several suitable ways to resolve the same issue.
Assign the right owner
An action owner should be able to coordinate the work and obtain the information needed. A deadline without ownership rarely clarifies the next step.
Define the review point
Agree how progress and effectiveness will be checked. This creates a useful follow-through rather than a list that is filed away after the meeting.
